CYP3A4

Most abundant hepatic CYP450 isoform; affected by both THC and CBD.

CYP3A4 metabolises a broader set of medicines than any other CYP isoform — including many antidepressants, immunosuppressants (tacrolimus, cyclosporine), statins, and azole antifungals. CBD and THC can both inhibit CYP3A4 to clinically relevant degrees. Patients on strong CYP3A4 substrates require additional monitoring or, sometimes, deferral of cannabinoid therapy entirely.
